The first edition of Cardiac Surgery: Safeguards and Pitfalls in Operative Technique was published 20 years ago. Its immediate success prompted it to be translated into Portuguese and Japanese. The third edition was translated into Japanese and Chinese. The book has become very popular among young surgeons in training, both at home and abroad.
This gratifying response to our work has encouraged us to pursue and launch yet another edition.
This new fourth edition has undergone extensive revision and has been updated to reflect the continuing changes in the practice of cardiac surgery and the impact of rapidly evolving new technologies. To accomplish this we have invited Dr. Abbas Ardehali to collaborate with us. He has written the new chapter on heart transplantation and has made numerous other contributions. Dr. Kwok Yun, who was involved with the third edition, has updated the chapters on surgery of the aorta and the aortic valve. Dr. Ramin Beygui has provided the section on endostenting of the thoracic aorta.
All chapters have undergone major scrutiny. Many new materials have been introduced. These include the surgical ventricular reconstruction for both left and right ventricular dysfunction, the management of porcelain aorta, and antegrade cerebral perfusion. The section on congenital heart defects introduces new procedures for pulmonary vein stenosis, updated approaches for Tetralogy of Fallot, the aortic translocation operation, and the Sano procedure. Several parts of the book have been completely rewritten, incorporating recent concepts and approaches into the text. Obsolete procedures have been deleted.
The book continues to be primarily intended for younger surgeons and those in training. However, it can also serve as a handy refresher on surgical technique for senior cardiac surgeons. Pediatric and adult cardiologists as well as perfusionists, nurses, and anyone involved in the care of cardiac surgical patients should find this work useful.
The format of the book follows that of the previous editions. The emphasis on pitfalls and errors in the performance of cardiac surgical procedures is denoted by highlighted subheadings, which are preceded by a hazard sign The reader is made aware of the mechanism of their occurrence, and appropriate recommendations are made regarding the avoidance of these complications and their surgical management. Points of particular importance are emphasized by special Nota Bene notations.
The text and the theme in this book do not pretend to be either encyclopedic or comprehensive. Less commonly performed procedures in cardiac surgery are excluded. The book focuses primarily on technical details. Relevant surgical anatomy is presented in a concise and practical fashion. This has been achieved with the aid of clear “telling” illustrations by Joanie Livermore in the first edition and continued in a masterly fashion by Tim Hengst. Tim has maintained the superb quality of the artwork which continues to be the hallmark of the success of the book. While some illustrations from the previous edition have been deleted, over 40 figures have been modified and 95 new ones have been added.
